Required Consent Under Data Protection Act, 2019

The Data Protection Act, 2019, requires that Ask A Doc Limited as a Data Controller and Processor to seek consent from Data Subjects before collecting and processing personal data. Ask A Doc Limited hereby seeks your formal consent to process your personal data solely for the purposes of conducting the Occupational Safety and Health (OSH) medical examination and submission of reports to DOSHS, the employer and to you in compliance to the Occupational Safety and Health Act, 2007 and its subsidiary legislations.

By clicking "I Agree" below, I confirm that:

  1. I understand that Ask A Doc Limited will collect, process and store my personal and OSH information in the Electronic Medical Records System and the Occupational Safety and Health Management Information System (OSHMIS) as required by the Directorate of Occupational Safety and Health Services (DOSHS).
  2. I consent to the use of my data for lawful OSH purposes, including conducting of medical examination, submission of medical examination reports, notification of occupational diseases, incident and accident reporting and statistical analysis as provided under the National OSH Policy, 2012, Occupational Safety and Health Act, 2007 and its subsidiary legislations, Work Injury and Benefits Act, 2007 and the Data Protection Act, 2019.

Note: Your consent is required to proceed with the Occupational Safety and Health medical examination as mandated by Kenyan law. If you do not provide consent, we will be unable to conduct the medical examination and generate the required reports for DOSHS.
